SYMPTOMS

When you try to use the Upload Document feature in a SharePoint Team Services document library, you receive the following error message:
This form is not available for this list, discussion board, survey, or document library.

If this problem persists, please contact your administrator.

Troubleshoot issues with Microsoft SharePoint.

CAUSE

The Upload.htm file has been deleted from the document library.

RESOLUTION

To resolve this issue, delete the document library and then recreate it. To delete the document library, follow these steps:
  1. On the page that displays the list, document library, or discussion board, click Modify settings and columns.
  2. Under General Settings, click Delete this document library.
To create a document library, follow these steps:
  1. In the top link bar, click Create. On the Create Page page, click Document Library.
  2. In the Name box, type a name for the document library. This field is required.
  3. In the Description box, type text that describes the purpose of the library. This field is optional.
  4. If you want a particular program to run when team members create a new documents in this library, select the type of blank document in the Template Type box.
  5. In the Navigation section, if you want a hyperlink to this document library to appear on the Quick Launch bar, click Yes.
  6. Click Create.

    A page that displays the default view of the new document library is added to the team Web site.

...and what do you do when you can't delete the library?? that part is broken too!
